TheraFascia (“TheraFascia,” “we,” “us,” or “our”) operates the website located at https://therafascia.com (the “Site”). This policy explains what information we collect from visitors to the Site, how we use it, and the choices available to you.

TheraFascia is a wellness solution and is not intended to diagnose, treat, cure, or prevent any disease.

Information you provide directly. When you submit the “Become a Founding Clinic” application form on this Site, we collect:

• Your first name, last name, work email address, and phone number
• Your clinic name and clinic website (optional)
• Your role (e.g. PT clinic owner, chiropractic clinic owner, employed therapist)
• How your clinic operates (e.g. cash-based, mixed, insurance-based)
• The proportion of your caseload involving chronic musculoskeletal (MSK) issues
• Whether and how your clinic uses hands-on manual therapy
• The number of clinicians actively seeing patients at your clinic

We use this information to evaluate whether your clinic is a fit for the Founding Clinic program and to respond to your application.

Information collected automatically. When you visit the Site, certain information is collected automatically through cookies and similar technologies, including:

• IP address and general location (city/region level)
• Browser type, device type, and operating system
• Pages visited, time on page, and referring URL
• Interactions with ads that led you to the Site

Campaign source data. If you arrive at this Site through a paid advertisement or marketing link, we capture the associated campaign parameters (UTM parameters) and store them alongside your application so we can understand which channel referred you.

Advertising and analytics technologies. We use third-party tools — including Meta Pixel and Google Tag Manager / Google Analytics — to measure the performance of our advertising and understand how visitors use the Site. These tools may place cookies or similar identifiers on your browser and may share information about your visit with the providers of those tools (Meta Platforms, Inc. and Google LLC).

We use the information described above to:

• Evaluate and respond to Founding Clinic applications
• Operate, maintain, and improve the Site
• Measure and improve the performance of our advertising campaigns
• Communicate with you about TheraFascia, including by email, where you have provided contact information for that purpose

We do not sell your personal information to third parties for their own independent marketing purposes.

3. Advertising Cookies and Your Choices

This Site uses advertising technology (including Meta Pixel) that begins collecting certain browsing information, such as pages viewed and general device/browser information, when you land on the Site.
